Valley Medical Group Practices Join Federal Program To Improve Primary Care

RIDGEWOOD, N.J. -- Valley Medical Group has announced that 20 of its 23 primary care practices in New Jersey have been selected for inclusion in the federal Comprehensive Primary Care Plus program, a partnership designed to provide improved access to quality health care at lower costs.

The program includes the Centers for Medicare & Medicaid Services (CMS), commercial health plans, self-insured businesses and primary care providers. Valley Medical Group’s practices are among nearly 2,900 primary care practices nationwide participating in Comprehensive Primary Care Plus, also known as CPC+.

“A robust primary care system is essential to achieve better care, smarter spending and healthier people,” said acting CMS administrator, Patrick Conway. “For this reason, CMS is committed to supporting primary care clinicians to deliver the best, most comprehensive primary care possible for their patients.”

The CPC+ program aims to improve patients’ experiences in primary care by increasing access to physician practices, strengthening ongoing care management and increasing engagement between patients and their medical care providers.

For patients, this means more communication from their physicians offices, better engagement for patients and caregivers in managing their own care and individualized, enhanced care for patients with multiple chronic diseases and higher needs.

“The CPC+ program will support primary care doctors and clinicians to spend time with patients, serve patients’ needs outside of the office visit and better coordinate care with specialists,” said Dr. David Strassberg, chair of Primary Care Services for VMG. “It is all about making sure we are providing the right care at the right time and in the right setting, while enhancing communication and the coordination of care.”
